SERVICES
At La Casa, Inc., we offer Case Management and/or Service Facilitation of Consumer-Directed Services to the DD/ID (CL & FIS), EPSDT, CCC Plus and EDCD Waiver recipients through the Division of Long-Term Care Department of Medical Assistance Services. This assures continued appropriate and adequate services to all individuals. Activities may include counseling or training personal attendants or individuals regarding their responsibilities as an employer, making requests to KEPRO, Managed Care Organizations (MCO’s)  (Aetna, Optima, Magellan, Va. Premier and United HealthCare) or Case Managers at local Community Service Boards (CSB’s) for adjustments to CSP/Plans of Care and discussing needs for additional care for the individual and requests made. Through our strong commitment to human service programs we also provide Group Home/Agency Start-Up Guidance and Training as well and the writing of Policy and Procedures and Human Rights Plans.

​CONSULTANT & TRAINING SERVICES

​La Casa, Inc. offers development and initial writing of Policy and Procedures and Human Rights plans for providers (new and current), discussion and review of documentation requirements (for Provider and/or Staff), Record Maintenance and Review, Staff Enhancement/Training in provided services and appropriate processes. Individual and Group Training is provided.

​CASE MANAGEMENT / SERVICE FACILITATION

​Case Management services coordinates and sets up activities designed to assist individuals with needed medical, psychiatric, social and educational, vocational, residential and other supports essential for living in the community.
Service Facilitation assures the setup, service delivery and appropriateness of Consumer Directed (CD) Services to those residing in the community. Activities include the counseling and training of Employers of Record (EOR’s) on responsibilities as an employer and their hired Personal Attendants to provide care to individuals in their homes and in the community as they desire.

​GROUP HOME EMPLOYER / EMPLOYEE TRAINING

​We will train and evaluate your staff/employees on the completion and writing of monthly, quarterly and progress reports and Individual Service Plans (ISP’s). This can include training on policy and procedures and human rights plans, individual, staff and family interactions and data collection.
​
​COMMUNITY RESOURCE TRAINING

​Our team will work with you and/or your staff to locate the appropriate resources to accommodate your needs as well as the individual(s) receiving services.

​​WAIVER SERVICES TRAINING

​This service is for Congregate, Respite, and Day Support providers of services. We will go over the developed policy and procedure manuals and/or human rights policies for people interested in a career in this human services industry.
​
​CAREGIVER TRAINING

​This training will help you understand what is necessary to care for individuals residing in private homes or group home settings. We will also cover Staff Orientation, Policy and Procedures and Human Rights by helping you formulate a care plan for those dependent upon you for their care.
